I'm applying for a job at my local Co op and I have an application form that I have to send off, but do I also send my CV and personal statement?

Only if they’ve asked you to include them.

At least, also send the CV. Particularly if you've done things that also would help you at the co-op.

I'm not a fan of overwhelming an employer with information, but I'm also a fan of representing yourself in the ideal light possible. If you fill out all fields of an application, but can say more about your experience with other things you've done, then send both.

Why not? A supportive covering letter and CV could give additional information regarding your experience and personality. Worst scenario is that it will end up being discarded but they’ll still have your Application Form.

it depends what the job is - if it is a part time/full time basic shop work job, just do the form.

if it is a manager/higher level job, then include more information.
